(1) The Death Certificate was never filed. (2) The Death Certificate was file, but it was lost in the paper mill. (3) Death record has not been transferred to Missouri State Archives (over fifty years old). (4) The person died in a out of state area hospital: (Keokuk, Lee County, Iowa (2 hospital before 1940's); Quincy, Adams County, Illinois (hospital before 1940); Hospital, Kirksville, Adair County, Missouri (before 1940); Davis County Hospital, Bloomfield, Davis County, Iowa (circa 1947); Van Buren County Hospital, Keosauqua, Van Buren County, Iowa (circa 1951), and Scotland County Memorial Hospital, Memphis, Scotland County, Missouri (after 1950)). The above reason are all dependance on time frame of death.2
